The Patient Portal is your secure online platform to manage your health information and communications with Angel Longevity Medical Center. Our comprehensive online system can provide you with all of the following critical information:

Test results: Access your lab results and other diagnostic tests as soon as they are available.
Important communication: We will notify you through the portal if critical results need immediate attention. You can also use your account to send and receive routine communications regarding your customized medical program.
Medical documents: Download your medical history, prescriptions, and any documents shared by your functional medicine doctor, Dr. Anju Mathur.
Appointment management: Schedule, reschedule, or cancel your appointments and receive reminders.
Prescription refills: Request prescription refills securely.

Once scheduled for your initial visit, you will receive instructions on how to log in and open a patient account.

Your First Visit: What You Need to Know

Angel Longevity Medical Center is committed to providing personalized, integrative care to meet your unique health needs. Whether you are visiting us for hormone therapy, nutritional testing, or another specialized service, we want to ensure you are fully prepared for your first appointment. Below are key details to help you navigate your visit:

Complete Your Medical History – Before your initial consultation, please fill out your medical history form in the Patient Portal. If you need assistance, our team is happy to help.
Allow Adequate Time – Many of our patients have been searching for solutions to long-standing health concerns. We take a comprehensive, team-based approach to your care, which requires time and thorough evaluation. Your first visit will take approximately three hours.
- Your consultation with the doctor may last up to one hour.
- You will also meet with your Program Consultant, Lab Technician, and Treatment Coordinator to discuss your care plan.
- Any necessary tests that can be performed immediately will be conducted during this visit to avoid delays in analyzing results.
- Your Treatment Coordinator will outline the next steps, including scheduling and coordination for your personalized functional medicine program.

This is a comprehensive, in-depth process designed to address your health concerns at the root level. Do not confuse it with routine or conventional medical checkups that take 15 to 30 minutes. We take your care seriously and encourage you to do the same by investing your valuable time and attention in this total wellness review.

Accessing Test Results – We will post your lab results securely on your Patient Portal as soon as they become available. We will contact you directly only for critical findings.
Appointment Policy – Please confirm your appointment in advance and note our 48-hour cancellation policy. If you need to reschedule, kindly provide us with notice at least 48 hours before your appointment time to avoid being charged.
